when to use “in + place”, “at + place”

in the hospital or in hospital: the person is a patient at the hospital. For example, He is in hospital right now. (= He is being treated as an in-patient.)

at the hospital: the person is just on the premises. For example, He is at the hospital right now. (= he is a visitor, outpatient, or staff.)
